FMCSA Certified Medical Examiner
Dr. Howard Fern, D.C., FIAMA, CME
FMCSA Certified Medical Examiner · DOT Physical Phoenix
Dr. Fern brings over three decades of professional experience to every exam. He earned his chiropractic degree from Southern California University of Health Sciences in 1993 and holds a BS in biology from the University of Hartford, where he also worked as an athletic student trainer.
He's a Certified Medical Examiner (CME) through the FMCSA for DOT/CDL physicals, and is certified for drug testing, breath alcohol testing, hair follicle testing, and respiratory fit testing. Dr. Fern has also served as team chiropractor for ProSports Chiropractic, the Arizona Cactus Football League, and local high school lacrosse teams.
His approach: “Hands on, high tech, modern treatment with personalized, old-fashioned caring.” You're not being rushed through a generic urgent-care visit — you're seeing an experienced professional who understands what commercial drivers need.
A DOT physical is a medical examination required by the Federal Motor Carrier Safety Administration (FMCSA) for commercial motor vehicle drivers. It's used to determine whether you meet the federal physical qualification standards needed to safely operate a CMV.
Our certified medical examiner evaluates your health against established FMCSA standards — some non-discretionary (like vision, hearing, and seizure history) and some left to the examiner's clinical judgment. So long as you're forthcoming about your medical history, our examiner will work with you through the process and explain exactly what's needed for certification.
Our medical examiner will evaluate you according to applicable DOT/FMCSA requirements — we never guarantee a specific outcome.
If you operate a commercial motor vehicle — whether you're a CDL driver, an owner-operator, or a fleet or company driver — you likely need a current DOT medical card to stay legal and on the road. This generally includes drivers who:
Not sure if your specific role requires a DOT medical exam? Give us a call or review the applicable FMCSA requirements — we're happy to help you figure out what you need.
Based in Gilbert, our office is a convenient stop for commercial drivers throughout Gilbert, Mesa, Chandler, Tempe, and the greater Phoenix metro area.
A little preparation goes a long way. Here's what to have with you at your appointment:

The typical exam takes approximately 30 minutes, although actual time can vary depending on your health history and individual circumstances.
$75 cash or $85 card for an all-inclusive DOT/CDL physical exam, which includes your free laminated medical card.
Yes. Appointments are preferred so we can minimize your wait, but walk-ins are always welcome during office hours.
No appointment is required — walk-ins are welcome. Scheduling ahead simply helps us have you in and out as quickly as possible.
Bring a valid photo ID, a list of your current medications, glasses/hearing aids if you use them, and any relevant medical records (like a CPAP compliance report or recent labs). See our full What to Bring checklist above.
Certification periods vary based on your medical circumstances and the outcome of your exam. Our examiner will let you know your specific recertification date after your visit, and we'll send free reminders as it approaches.
Our examiner reviews your medical history, checks your vision and hearing, takes your blood pressure, and performs a physical exam covering your cardiovascular, respiratory, neurological, and musculoskeletal health, plus a mandatory urinalysis (this checks for protein, blood, and sugar — it is not a drug test). You'll leave with your Medical Examiner's Certificate and laminated medical card.
Yes. We can email your health history forms ahead of time so you can complete them at home or in the office before your exam.
Yes. We offer DOT drug and alcohol testing, including breath alcohol testing and hair follicle analysis. See our Drug & Alcohol Testing service for details.
Yes, we also perform non-DOT drug testing, including rapid screens, for employers and individuals who need testing outside of FMCSA requirements.
Yes. We can fax or email your exam documentation directly to your employer when requested.
Blood pressure is one of the areas our examiner evaluates as part of your exam. Findings can affect certification, and our medical examiner will evaluate you according to applicable DOT/FMCSA requirements — including the possibility of a shorter certification period for monitoring. We don't promise a specific outcome, but we'll walk you through exactly what your results mean.
Many drivers with managed conditions like diabetes or sleep apnea are certified every day. Eligibility and certification length depend on the applicable FMCSA requirements and your individual circumstances, so bring relevant documentation (like recent A1c labs or a CPAP compliance report). So long as you're forthcoming about your history, our examiner will help guide you through the process — without guaranteeing a specific outcome.

“Best place to go in the Valley for your DOT Physical. Dr. Fern is very professional and treats you like a person, not just a number. I will never go to one of those other places again!”

“Best chiropractor ever! Dr. Fern takes his time at the beginning of each adjustment to actually see what's going on with your spine, before adjusting you, rather than just doing the same adjustment every time like a robot, like many other chiropractors seem to do. I've had chiropractic work done on me pretty regularly for the last 20 years or so, and have only been a patient of Dr. Fern's for about a year, and I can say his adjustments last longer than any other chiropractor I've been to. He's phenomenal at his craft and can really tune into what your body needs. 10 out of 10 for sure!”

“I arrived at A Plus Chiropractic just 10 minutes early. Scheduled my DOT (health card) appointment with them — after all, it was half the cost. But the experience was 100x more satisfying than any urgent care or clinic I've ever been to. With a history of high blood pressure and needing to pass for my livelihood, I entered a bit nervous, and the city's road construction traffic didn't help. The office lobby area was inviting and instantly calming — I was greeted with a hello and a huge smile, and processed in gently but professionally. So impressed with Dr. Fern! My initial blood pressure reading was too high, as I expected, but Dr. Fern has these abilities and simple techniques to calm and relax a patient's mind and body within minutes. Just amazing. I left the Gilbert office doing my happy dance. Thanks a million — see you again.”
