STEP 1: Make an appointment

STEP 2: Health History

Complete the Medical Examination Report Form before your appointment. A health history link will be sent to you once an appointment is made. (You can also complete this form at our office).

STEP 3: DOT Physicals

Complete your Physical with our FMSCA Certified Medical Examiner. Please avoid smoking or drinking caffeine/ energy drinks before your exam.

STEP 4: Receive your Paperwork

• Medical Examination Report Form (long form)
• Full Size Medical Examiner’s Certificate (short form) and laminated wallet size medical certificate
• Your forms will be securely stored at our office.

STEP 5: Submit Paperwork

You must submit the Medical Examiner’s Certificate to your State Motor Vehicle Department.

STEP 6: Reminders

Our office will schedule your reminder notification via e-mail or text so you do not miss your next exam.
